Not that I know of. If there is medical evidence this is cause of death I don’t know how else that can be explained? You couldn’t argue mistake / incompetence on that.

I can’t understand how people say scapegoat . Just today’s updates has me thinking GUILTY . Injecting air in the blood stream . collapsing happening on her night shifts then she moved to days and it followed but stopped at night . Injecting insulin . thats not hospital failings surely
Just echoing what others have said really but drug errors could happen but I’d struggle to see how it would occur continually as a mistake on the one ward over a short period multiple times. In my 12 years qualified I give insulin most days and I’ve known 3 nurses in this time who’ve administered too much in error, mainly due to prescriber’s poor handwriting on the drug administration chart making it appear like a different dosage (the patients were fine and the nurse alerts the manager that they’ve made an error). I think electronic kardexes are more a thing now in hospitals to eliminate the chance of error. Drug errors do happen and it’s registered nurses worst nightmare but it’s reported and rectified, you reflect on your practice etc and tend not to make that mistake ever again!
Air embolisms are so rare within paediatrics! I’ve never come across it. You would have to push a lot of air in? Naturally you get some bubbles in infusion lines and that’s fine but goodness that’s made me feel a bit sick
Yeah us gingers need upto 20% more anesthetic. I need the full 20% more. Its happened a few times I'm given what they expect will be enough for someone of my size but it never is enough.

Tell me about it! The trust (you’d hope) would be sending out education/training on the topic to prevent it happening again. You’d be hyper alert to it
If you were wanting to get away with serial killing in that kind of setting I imagine you would choose different methods and patterns to try and not make it noticeable.
