Yep she's broke the NMC code of conduct once again, patient confidentiality. They used to be called incident reports and you could grade them on the severity of said incident. I think she's took it in case she needs it for evidence. Lack of equipment, not enough staff.
Plus all those near misses and deaths should have a datix completed. Very bad practice if not completed.
