The only people who exist or really register in his mind are those who compliment or notice him.Also, anyone who has volunteered in a soup kitchen / homeless charity will tell you it’s not all gentle loveliness and grateful clients telling you how wonderful you are - it can be a really challenging environment, things can get uncomfortable and even violent, because obviously you are dealing with people who may have drug or alcohol problems and often quite serious mental health issues too.
