In the UK you legally have to take a minimum of 20 mins after working 6 hours, and it must be during your shift, not tacked onto the end of it.
So, starting at 9am you must have at least 20 mins at 3pm.

Quite often do a 12 hour shift with no break. Sometimes on a 13 hour shift I get my break 10 hours in.
Because other people need you to do stuff for them or answer a question at 4.59pm?
