In the UK you legally have to take a minimum of 20 mins after working 6 hours, and it must be during your shift, not tacked onto the end of it.
So, starting at 9am you must have at least 20 mins at 3pm.

Hospitals all over the country break the law daily 🤣
Quite often do a 12 hour shift with no break. Sometimes on a 13 hour shift I get my break 10 hours in.

That falls under section 21 of the WTR (Special Cases, a.k.a 'derogations') where your delayed break is classed as compensatory rest.
