I don’t get it

I think it’s because she looks so young for 95?

It's the bit at the top "Her son's a doctor" ie: why was she 95 before she first went on a plane. Her doctor son could presumably have paid for her to go years ago.

Ah I see! I thought it was an American meme and they could tell her son was a Doctor because she looked so fit and well and didn’t have to pay for healthcare
