It's the bit at the top "Her son's a doctor" ie: why was she 95 before she first went on a plane. Her doctor son could presumably have paid for her to go years ago.

It’s a parody of a sketch on “Come fly with me”, a horrible old lady was treated like a celebrity because of the assumption that it was her first flight and her son was a doctor.
