Any nurses/midwives? - interview help

I have an interview coming up - I know one of the questions from last year was something like 'tell us about a project/task you've successfully implemented' or 'tell us about a time you have improved practice'.

My heads gone. Aren't these questions essentially the same? Are they about change management? How would you answer?
 
Hey! Yeah these questions are basically the same... they just want you to talk about something you’ve successfully brought in to show you can adapt and are always looking at changing practice!
I have been asked this for both of my interviews and I’ve also been on the interview panel for band 5 nurses too. In mine I mentioned that we were having a lot of falls, so got the staff to increase the rounding we did and noticed a reduction in falls! I’ve heard of other people say that they realised patients weren’t drinking much, so brought in a 10 and 2 o’clock drinks round to encourage nutrition and hydration!
I think people over think it. Anything small can be a massive change! Hope the interview goes ok for you :)

I think I've been over thinking it. I did introduce a communication sheet to completed on admission eg referrals made, investigations ordered etc - to be kept on front of each patients file for ease of reference for MDT to see what's been done/needs done or what results are pending. Is this something that I could use?
