Also, GPs have operated as 'ad hoc social workers' for a very long time. As have so many other health and social care professionals because there IS an intersection. Although modern health care points to specialism, that doesn't apply so much to integrated general practice because (thank god) a lot of GPs are recognising that health equals more than prescriptions and clinical treatments. They just need more money (and better NHS structures) to do this well.
An example: GP diagnoses 2TD. Practice nurse monitors overall health. GP tweaks meds. GP or nurse makes the referrals to OT, social care, podiatrist, dietitian, eye clinic, vascular team, cardiac team, physio and local exercise classes, diabetes groups etc. Practice notes collate all of this.
